Paperwork to Play
Attention Parents/Guardians and Athletes:
As a reminder, the Paperwork to Play process must be completed for each new school year.
In order to participate in sports at RBHS, each athlete MUST complete the following process each year:
- Print blank Health History and Physical forms by clicking HERE
- Take the completed Health History form and blank physical form to doctor (MD, DO, PA or NP is acceptable). Doctor must complete, sign, date, and stamp the physical form. If you are unable to get an appointment with your child's pediatrician, please visit your nearest CVS Minute Clinic.
- Complete on-line AthleticClearance.com for the 2026-27 school year. Use the instructions below to navigate the process. This includes important information for multi-sport athletes. Please use existing account if you are a returning athlete - do not create a new profile or account! New this year, you will be uploading your student's Health History and Physical forms directly into AthleticClearance.com
- Print and sign confirmation page from AthleticClearance.com
- Turn in signed confirmation page to the Athletics office (emails and copies not accepted).
- A parent/guardian of a new RBHS athlete must attend a mandatory in person informational meeting before the athlete will be cleared. This applies to all freshmen (even if they have an older sibling that has played sports at RBHS), transfers and current students being cleared for sports at RBHS for the first time. Athletes that were cleared for a sport during the 2025-26 school year are exempt from this meeting requirement. You are able to check your athlete's status on AthleticClearance.com.
Signed AthleticClearance.com confirmation page must be turned into the athletic office (emails/fax NOT accepted) by the designated date below to ensure an athlete is able to participate in tryouts.
Fall Sports - June 11, 2026 (office closes at noon)
Winter Sports - October 30, 2026
Spring Sports - January 29, 2027
If you need to add an additional sport to your student's profile, please click here for directions. PLEASE DO NOT CREATE A NEW ACCOUNT OR PROFILE FOR YOUR STUDENT! Students may not try out for a sport if they don't show as "cleared" for that sport on their AthleticClearance.com profile.
All Athletic Physicals are valid for 12 Months from the date the physical was completed and must be valid for the length of the entire sport season (see chart below).

*The above dates are determined by the section finals dates on the 2026-27 CIFSDS master calendar for each sport and apply regardless of student's grade or level of play.
- If your physical will expire in the middle of the season, you must have a new physical done before tryouts.
Ex: If your physical was done in July 2025, you will need a new physical for the 2026-27 school year which will be good for the entire year. If you had your physical in January 2026, your physical is good only for the Fall 2026-27 sport season.
- If you are a new student (new to RBHS/PUSD), you can get your physical from your old school and upload into AthleticClearance.com (as long as it’s not expired or will expire during the sport season). You still must complete all other Paperwork to Play steps listed above.

Student Insurance
Insurance is required for all students trying out for sports.
